چکیده

Methods Patients with severe AS had both pre-operative CMR and trans-thoracic echocardiography (TTE) within one week apart. From TTE, on top of the routine measurements, Zva was calculated as the sum of systolic blood pressure and the aortic valve continuous-wave Doppler mean gradient divided by the left ventricular stroke volume index. Patients were categorized into 2 groups according to their calculated Zva: (a) High Zva (≥4.5 mmHg/mL/m2), and (b) low Zva (<4.5). CMR parameters, namely left ventricle mass index (LVMI), ejection fraction (LVEF), enddiastolic volume (LVEDV), right ventricle end-diastolic volume (RVEDV) and left atrium volume index (LAVI), were compared between the 2 groups.

The Role of Index of Valvular-Arterial Impedance and Systemic Arterial Compliance after Aortic Valve Replacement

Mechanical valvular obstruction and reduced arterial compliance combine to increase left ventricular afterload in pati-nets with aortic stenosis (AS). 1) As a result of the recognition that valvular and arterial abnormalities both play important roles in determining the overall impedance to left ventricular ejection in AS, it is now clear that standard methods of quantifying valvular stenosis, ...

Impact of valvuloarterial impedance on 2-year outcome of patients undergoing transcatheter aortic valve implantation.

BACKGROUND Elderly patients with severe aortic stenosis undergoing transcatheter aortic valve implantation (TAVI) often have increased calcification and fibrosis of the aorta. Indices that account for the severity of valvular obstruction and systemic vascular impedance may better assess total left ventricular afterload. The aims of the present study were to evaluate changes in valvuloarterial i...

Does valvuloarterial impedance impact prognosis after surgery for severe aortic stenosis in the elderly?

BACKGROUND Valvuloarterial impedance (Zva) was introduced as a prognostic measure in patients with aortic stenosis (AS). However, it is unclear whether Zva has a prognostic impact on survival after surgical aortic valve replacement (AVR) in patients with severe AS with preserved ejection fraction (EF). METHODS We retrospectively reviewed 929 consecutive patients who had AVR. We investigated 1...

Valvuloarterial impedance does not improve risk stratification in low-ejection fraction, low-gradient aortic stenosis: results from a multicentre study.

OBJECTIVES In a multicentre series of patients with low-ejection fraction/low-gradient aortic stenosis (LEF/LGAS), we evaluated the prognostic impact of valvuloarterial impedance (Zva). BACKGROUND Zva in AS, a measure of global afterload taking into account systemic arterial compliance, has been proposed for risk stratification in paradoxical LGAS. We hypothesized that Zva could help risk str...

Association of myocardial deformation with outcome in asymptomatic aortic stenosis with normal ejection fraction.

BACKGROUND Current guidelines recommend intervention for symptomatic aortic stenosis, but the management of asymptomatic aortic stenosis remains controversial. As left ventricular global longitudinal strain (GLS) has been shown to predict cardiovascular outcome, we sought to find whether its use could guide the assessment of risk in these patients. METHODS AND RESULTS We prospectively followe...
